Before you come in, tell us how to look after you.

Most people who fill this in haven’t been to a dentist in years, and dread the thought. That’s who we built it for. Nine short questions, no wrong answers, and you can skip any of them.

Nothing here gets you a lecture. It gets read by Georgia before you arrive, so you don’t have to explain yourself twice.

When did you last see a dentist?

However long it’s been, we’ve seen longer. Honestly.

Are you in any pain at the moment?

This one changes how quickly we get in touch.

What worries you most?

Tap as many as you like. This is the bit that changes what we actually do.

Has something happened before?

Only if you want to. A sentence is plenty — it means you won’t have to tell the story in the chair.

What would help you feel in control?

Pick anything that sounds right. We’ll put it in your notes and actually do it.

Anything physical we should know?

Things that make a visit harder, so we can plan around them.

What would you love to be able to do?

The reason underneath the appointment.

Georgia, our treatment co-ordinator, reads these herself. Tell her how you’d rather be contacted and she’ll stick to it.

What we’ll do for you
    Worth saying plainly

    You may not need any treatment at all.

    It’s the fear of what we’ll find that keeps most people away, so it’s worth being blunt: plenty of patients who haven’t been in a decade need nothing done. Mouths are very often in better shape than their owners fear.

    The one thing we suggest for everyone is a visit to the hygienist. A proper clean, and gums kept healthy, is what stops small problems ever becoming big ones — and for a lot of people that’s the only appointment they ever need with us.
