Airflow or scale and polish? How each works, and which your teeth need

The short answer: Airflow does most of what a scale and polish did, more comfortably and more thoroughly, which is why it is now our standard hygiene visit. Hardened tartar below the gumline still needs targeted scaling, so most visits are Airflow with a little scaling where it is needed, not one or the other.
A scale and polish is what most adults grew up with: a hygienist scraping tartar off with metal instruments, then buffing the teeth with a gritty paste in a spinning rubber cup. Airflow replaces the scraping and the paste with a warm spray of water, air and fine powder. Here is what each actually does, a side-by-side, and the honest cases where the old way still earns its place.

How each one works
Traditional scale and polish
Hand scalers and an ultrasonic tip chip and vibrate tartar off the teeth above and below the gumline. Then a rotating rubber cup with abrasive paste polishes the surfaces and takes off some surface stain. Effective on hardened deposits, less pleasant, and the paste cannot reach between teeth or under the gum.
Airflow with Guided Biofilm Therapy
Plaque is disclosed with a coloured gel so it can be seen. A wand then sprays warm water, air and erythritol powder over teeth and gumline, lifting soft plaque, early tartar and stain from every surface, including between teeth and around dental work. Targeted scaling follows only where hardened tartar remains.

- Stain: Airflow lifts tea, coffee, wine and smoking stain evenly across every surface. Polishing paste takes stain off the flat fronts of teeth and leaves the rest.
- Hardened tartar: this is where scaling still wins. Calculus that has set below the gumline needs an ultrasonic tip or a hand instrument. Airflow removes soft and early deposits, not old concrete.

When scaling is still the right tool
If it has been years since a clean, or gum disease has let tartar build below the gumline, the first visit is mostly scaling and may be a deep clean over more than one appointment, with Airflow finishing the job. That is not a failure of Airflow; it is the order of operations. Once the mouth is clean, Airflow keeps it that way, and the scraper rarely comes out again. Does Airflow whiten teeth?
It removes surface stain, which is often what people mean by whiter, and the difference after one visit can be striking on a tea or coffee drinker. It does not change the colour of the tooth itself. If your teeth are still darker than you want once the stain has gone, whitening is the next step, and a clean first makes whitening work better.
Bad breath, bleeding gums and the other reasons to come
Most persistent bad breath is bacteria in the biofilm along the gumline and between teeth, which is precisely where Airflow reaches and paste does not. Gums that bleed when you brush are inflamed from the same biofilm, and they stop bleeding within a couple of weeks of it being removed and kept off. Both improve more with Airflow than with a polish, because the cause is being cleaned rather than the surface being shined.
Is it worth the extra?
For a routine six-monthly clean on healthy gums, we think so, and most of our patients who have had both agree: same appointment length, more of the mouth cleaned, no scrape, and safe around any dental work you have. For a mouth that needs a deep clean first, the extra is not the point; getting the gums healthy is, and Airflow is the maintenance that follows. Questions people ask
Is Airflow better than a scale and polish?
For routine cleaning and stain, yes: more comfortable, more thorough, and safe around dental work. For hardened tartar below the gumline, targeted scaling is still needed, so most visits combine both.
Does Airflow remove tartar?
Soft plaque and early tartar, yes. Hardened calculus that has set below the gumline needs an ultrasonic tip or hand scaling, which we do on the same visit where needed.
Does Airflow whiten teeth?
It removes surface stain, which makes teeth look noticeably brighter, but it does not change the tooth's own colour. Whitening does that, and works better after a clean.
Is Airflow safe on implants and veneers?
Yes, and it is the recommended way to clean around them. Metal instruments and gritty paste can scratch porcelain, resin and implant surfaces.
How much more does Airflow cost than a scale and polish?

Can I still have a traditional scale and polish?
If your mouth needs scaling, you will get it as part of the visit. We do not offer a paste polish on its own any more because Airflow does the same job better.
